CRISIS CARD
To be carried by anybody who may experience extreme mental distress and needs to be able to have a say in their own care even when speaking is impossible. The card needs to be filled in when the person is in a calmer state perhaps assisted by a professional (CMHT care co-ordinator, support worker, psychologist etc.)
The poster can also be displayed in work places, for example, police stations, to get the officers used to the idea of asking for it if responding to a mental health crisis incident. Please co-ordinate with the emergency services in your area.
We do not charge for these or for P&P but any donations would be very much appreciated.
