 Logging good and bad experiences - Battle Scars service users only

A large number of people who self harm have had contact with a variety of services, e.g. GP, A&E or minor injuries, mental health services. Sometimes the help we receive is useful and sometimes it's inadequate. You can report and log such experiences with Healthwatch, the consumer champion for health and social care in the U.K. We are aware that some of our service users would find that too daunting and may prefer to log such experiences with Battle Scars. As an organisation we are growing fast and have already established connections in Leeds with NHS services, the police, other third sector organisations and Healthwatch. We aim to work together to improve the support provided to people who self harm and their families. 

If you wish to let us know about your experiences (please also include good ones!) please fill in the form below. 

If you need to complain, please follow each service's complaints procedure or log it with Healthwatch who can advise you on how to put an official complaint in.

    Any personal information will be removed if we use this as evidence of good or bad practice.
    Please indicate below if we have your permission to share this.

    For simple identification purposes. We will never disclose any of this information without your permission.
    If you are filling this on behalf of someone else, please indicate their age not yours.
    Male, female, gay, lesbian, trans etc.
    Specific date or period. Please be as specific as possible.
    e.g. Leeds or West Yorkshire We are aware treatment varies across the country
    Such as health services (GP, A&E, minor injuries etc), mental health services (CMHT, Crisis etc), education provider (school, college, university), 3rd sector (charities, support groups etc)
    Please include all facts such as location and times this happened as well as how that made you feel. Be specific which service you came in contact with (e.g. A&E at St. James's Hospital or CMHT St. Mary's House)
    We will only use this as a means to bring improvement or give credit where it's due. No personal information will be passed on.
    We might need to clarify some facts with you.
    We will never pass on your email address to any other organisation or agency unless you ask us to.
